High School Registration forms for Teens
The following forms are required for any teen who wishes to participate in any event sponsored by St. Joseph's Catholic Youth Ministry or held on any property owned by or affiliated with St. Joseph's Catholic Church or the Diocese of St. Augustine.
Required forms for Adult Volunteers
The following forms are required for any adult volunteer (over the age of 18 & out of High School) who wishes to participate in any event sponsored by St. Joseph's Catholic Youth Ministry or held on any property owned by or affiliated with St. Joseph's Catholic Church or the Diocese of St. Augustine.
In addition to these forms, the adult must be fingerprinted and cleared by the Diocese of St. Augustine.
